Ability Case Management provide an independent service with a duty of care to the client and their family members. Our aim is to support clients achieve their full potential after injury.

What is Case Management?
“Case Management is a collaborative process, that assess, plans, implements, coordinates, monitors, and evaluates the options and services required to meet individual’s health and wellbeing, education and/or occupational needs, using communication and available resources to promote quality, cost effective and safe outcomes.”

The key areas we cover are: collecting background information, measuring current health and wellbeing, monitoring daily activities, establishing and maintaining a rehabilitation programme, dealing with accommodation and installation of any additional equipment, providing ongoing care and support, and organising any finance requirements.
